Introduction of Updated NIN Medical Evaluation (Screening) Form

Update
Following consultation with the AMS Working Group and several NIN and NSO doctors, the NIN Athlete Medical Screening form has been reviewed and updated. The aim of this work has been to modernise the current form, introduce greater flexibility in how it can be used, and provide a consistent base template across NIN, NSOs and Major Games environments.
While the updated form provides a standardised foundation, doctors may still use additional forms or assessments that sit separately in AMS (e.g. LEAF Q, SCAT6 etc) as part of their own screening processes where required.
Development has also been undertaken within the current technical constraints of the AMS platform (i.e. current inability to link external pathology and radiology providers).
Key changes
TERMINOLOGY UPDATE
The form now uses the term “Medical Evaluation”, with “screening” retained in brackets for the time being to maintain familiarity.

SHORT AND LONG VERSION OPTIONS
Two versions of the evaluation are now available, accessed within the same form. A tick box at the start of the form allows the user to select either the short or long version depending on the needs of the sport or clinical context.
Short version – suggested use
The short form may be useful for:
- Pre-travel or pre-event medical information (e.g. Commonwealth Games)
- Situations where a shorter review is appropriate in place of a full annual screening ie may consider alternating yearly between short and long versions
- Collecting baseline information for non-NSO/NIN athletes attending camps
Typically, the athlete completes the form and the doctor reviews it. If no issues are identified, a face-to-face consultation may not be required. If concerns arise, the athlete can be referred for a more comprehensive assessment. This version is primarily intended as a way to capture the current situation on injuries, illness, medication, allergies.
Long version (full evaluation) - suggested use
The longer form is broadly similar to the previous screening form but has been updated and refined. It is intended for:
- Entry into the NIN/NSO system
- Annual or Alternate-year screenings (with short form)
- Situations requiring a more detailed health review
Some of the new highlights:
Cardiac: Cardiac screening questions reviewed with sports cardiologists (included in the long version only) with inclusion of indigenous specific considerations.
Concussion: Expanded concussion history questions.
Mental Health: A targeted mental health section, with an option to activate a linked questionnaire (SMHAT-1).
Linked form access: Doctor-only fields to allow additional entries into AMS injury and illness (and medication) records during the screening process without losing your place/entered data.
Examination Findings: have removed individual anatomical/regions boxes and replaced with an open text box for relevant findings… no more femoral pulses!!
File Uploads: Ability for athletes to upload supporting documents, such as Medicare immunisation records or care plans. Which also allows for the individual vaccination date section to be removed and replaced by current medicare immunisation form upload.
Screening Dashboard: The short and long NIN evaluations, along with the ALPHA and NIN Winter screenings, will also feed into the screening dashboard, allowing NSOs, NINs and Major Games teams to review athlete screening status more easily.
What happens next
- The updated NIN Medical Evaluation (Screening) will go live from Wednesday 27th May, 2026.
- All previous NIN Medical Screening data will remain within the form and therefore will be accessible within the AMS through standard reporting and dashboard views.
- Doctors are encouraged to review both versions of the new form in AMS and consider how the short version may be incorporated into their workflows.
