How to Implement and Use the Updated NIN Medical Evaluation (Screening) Form

Following consultation with the AMS Working Group and several NIN and NSO doctors, the NIN Athlete Medical Screening form has been reviewed and updated. The updated structure is intended to provide greater flexibility in how it can be used while still maintaining a consistent national foundation across NIN, NSO and Major Games environments.
Two versions of the evaluation are now available within the same AMS form.
This change provides significantly greater flexibility in how the form can be implemented across different programs and environments.
Important Implementation Guidance
It is up to each NIN and NSO to determine how they wish to implement and use the increased flexibility now available within the NIN Athlete Medical Evaluation (Screening) form.
For this form to work, it is important that athletes are clearly guided and instructed regarding which version of the form they are expected to complete. When they log into their AMS profile and open the form, the first question they will need to answer is which version they are to complete:
This guidance may be provided through:
- Team communications,
- Appointment booking processes,
- Pre-camp information,
- Doctor or medical staff instruction, or
- Internal NIN/NSO procedures.
Long Version (Full Medical Evaluation)
The long version remains broadly similar to the previous screening form, however it has been reviewed, refined and updated.
Suggested Uses
The long version is intended for:
- Entry into the NIN/NSO system,
- Annual medical evaluations,
- Alternate-year screening models used alongside the short form,
- Situations requiring a more detailed health review or consultation.
Typical Workflow
No change has been suggested to the current workflow for the Full Medical Evalation process:
- The athlete completes the form, and
- The doctor reviews the responses in a face-to-face consultation.
Short Version (Pre event / games / camp / annual review)
The short version is primarily intended as a streamlined way to capture current information relating to:
- Injury,
- Illness,
- Medication,
- Allergies, and
- Relevant current health concerns.
Suggested Uses
The short version may be useful for:
- Pre-travel or pre-event medical information collection (e.g. Commonwealth Games),
- Situations where a shorter review is appropriate instead of a full annual screening,
- Alternate-year approaches where short and long versions are alternated,
- Collecting baseline information for non-NIN/NSO athletes attending camps or programs.
Typical Workflow
Typically:
- The athlete completes the form,
- The doctor reviews the responses, and
- If no issues are identified, a face-to-face consultation may not be required.
IMPORTANT: Where concerns or risk factors are identified, the athlete can be scheduled for a more comprehensive assessment and / or be required to complete the long evaluation form.
How to ask athletes who have completed the "short version" to return to their entry and complete the "long version"
Typically, their are two reasons an athlete may be required to return to their AMS profile to enter more data into the NIN Athlete Medical Evaluation (Screening) form:
- The athlete incorrectly completes the "Short Version" (Pre event / games / camp / annual review) when they were required to complete the "Long Version" (Full Medical Evaluation), or
- The reviewing Doctor notices something in the responses to the "Short Version" (Pre event / games / camp / annual review) that warrent a need for the athlete to complete a more thorough evaluation and thus complete the "Long Version" (Full Medical Evaluation).
In both situations, the instructions to the athlete are the same. Do not ask them to complete a new entry, rather instruct them to:
- Log into their Teamworks AMS App,
- Click “Activity” and scroll to their record,
- Change the answer to the first question and scroll through the form to complete the “new” questions that become visible.
Recommended Next Steps for Doctors and Medical Teams
Doctors and medical teams are encouraged to:
- Review both versions of the updated form within AMS,
- Consider how the short version may fit within existing workflows,
- Determine how athletes will be instructed regarding which version to complete, and
- Develop local processes that align with the needs of their sport, athletes and medical resources.
