How to ask athletes who have completed the "short version" to return to their entry and complete the "long version"
When would this happen:
Typically, their are two reasons an athlete may be required to return to their AMS profile to enter more data into the NIN Athlete Medical Evaluation (Screening) form:
- The athlete incorrectly completes the "Short Version" (Pre event / games / camp / annual review) when they were required to complete the "Long Version" (Full Medical Evaluation), or
- The reviewing Doctor notices something in the responses to the "Short Version" (Pre event / games / camp / annual review) that warrent a need for the athlete to complete a more thorough evaluation and thus complete the "Long Version" (Full Medical Evaluation).
Instructions to provide athletes
In both situations, the instructions to the athlete are the same. Do not ask them to complete a new entry, rather instruct them to:
- Log into their Teamworks AMS App,
- Click “Activity” and scroll to their record,
Change the answer to the first question and scroll through the form to complete the “new” questions that become visible.
