NUTRITION RECORD
CENTRALISED DATA ENTRY METHOD
Practitioners will now use the "Nutrition Record" form as the primary entry point for documenting:
- Individual consultations,
- Group Delivery,
- Documenting notes from discussions with other practitioners, and
- Linking other data relevant to a consultation.
Key Improvements
- The updated workflow enables practitioners to document relevant consultation notes while also linking additional data sources—such as Body Composition assessments—within a single, integrated record.
- This approach enhances clinical documentation and provides a more complete and detailed view of the athlete's condition and progress.
Accessing the Form
The form is readily available as a quick-link under "Favorite Events" on the landing page. To access it:
- Hover over the tile, select "Enter Data," and locate the relevant athlete.
HISTORICAL CLINICAL FLAGS AND RECORDS
This section provides a comprehensive summary of historical clinical flags that may offer valuable context for your consultation. These include important considerations such as risk factors, allergies, and other flags relevant to clinical decision-making.
The clinical flags list is conditionally hidden and will become visible when the practitioner selects "Yes" in response to the preceding question, indicating the intention to add a new clinical flag.
Any flags that are ticked will automatically populate in all subsequent new Nutrition Record forms, ensuring consistency and continuity of care.
VIEWING SHARED CONSULT DATA FROM OTHER DISCIPLINES
Each discipline will have its own version of the new Consult Form.
All practitioners across disciplines will have the ability to selectively share relevant information with other disciplines.
This section enables clinicians to review data entered BY their discipline and those shared WITH their discipline while conducting their consultation, ensuring better collaboration and continuity of care.
PRE-CONSULT TAGS
By completing these indicators, practitioners contribute to periodic, discipline-wide reviews aimed at supporting effective practice evaluation and workload management.
The type of record selected determines which tags and notes fields are displayed. Practitioners are encouraged to complete these fields to ensure accurate categorisation and meaningful insights across the discipline.
- Individual delivery - one on one consultations with athletes.
- Group delivery - sessions/ presentations delivered to more than one athlete at a time.
- Other Notes - discussions / meetings held with other practitioners.
NUTRITION RECORD NOTES
This section is designated for documenting consultation notes, including key observations, assessments, and recommendations made during the session. It ensures a structured record of the client’s nutritional status, dietary interventions, and follow-up plans.
Consultation notes have been expanded to use the SOAP style format within the "Nutrition Record" form. Examples of what can be included within each text box are as follows:
Subjective (Sports Nutrition Focus)
- Athlete’s Goals/Concerns: What are the athlete’s nutrition-related goals (e.g., improving performance, weight management, muscle gain, recovery, etc.)?
- Dietary Habits: Description of the athlete's current eating patterns (e.g., timing of meals, pre/post-training nutrition, hydration, meal frequency).
- Energy Levels: How the athlete feels throughout the day, before, during, and after exercise.
- Training Schedule: Frequency, duration, and intensity of training sessions (affects energy requirements and meal timing).
- Symptoms/Challenges: Issues like fatigue, digestive discomfort, or difficulty maintaining weight goals or muscle mass.
Objective (Sports Nutrition Focus)
- Body Composition: Measurements of weight, body fat percentage, lean muscle mass, and any changes observed over time.
- Performance Metrics: Performance data (e.g., times, distances, strength, endurance, speed, power) relevant to the athlete’s sport and goals.
- Hydration Status: Urine colour, signs of dehydration, fluid intake during and after exercise.
- Lab Results: Relevant blood work or biomarkers that relate to athletic performance (e.g., iron levels, electrolyte balance, blood glucose, etc.).
- Supplement Use: Any nutritional supplements or ergogenic aids being used, including types, doses, and timing.
Assessment (Sports Nutrition Focus)
- Nutrition-Performance Connection: How current nutrition practices might be affecting performance (e.g., poor fuelling leading to fatigue or slow recovery).
- Dietary Gaps or Surpluses: Evaluation of nutrient intake (calories, macronutrients, micronutrients) relative to the athlete's energy expenditure and goals.
- Injury Recovery/Nutrition Needs: If applicable, how nutrition is supporting recovery or healing from an injury.
- Behavioural Factors: Any psychological or environmental factors influencing eating behaviours, such as stress, motivation, or team dynamics.
Plan (Sports Nutrition Focus)
- Nutrition Strategy: Adjustments to macronutrient intake (carbs, protein, fat) to meet energy demands, muscle recovery, and performance goals.
- Meal Timing: Recommendations for meal/snack timing around training (pre-workout, post-workout, and throughout the day).
- Hydration Protocol: Plan for maintaining hydration, including pre, during, and post-exercise fluid strategies.
- Supplements/Ergogenic Aids: If necessary, recommend safe and evidence-based supplements (e.g., protein powders, creatine, electrolytes) to support goals.
- Education and Support: Provide education on sports nutrition principles, fuelling strategies, and recovery practices.
- Follow-Up Plan: Regular check-ins on progress, reassessment of goals, and adjustments as necessary based on performance and feedback.
This format ensures a comprehensive approach to sports nutrition, focusing on the athlete’s goals, training demands, and performance.
POST-CONSULT TAGS
Accurately tagging the topics covered within the record helps create a structured and searchable database, enabling practitioners to identify trends and patterns across the discipline. This systematic approach supports evidence-based practice, facilitates research, and enhances the development of targeted interventions to improve patient outcomes on a broader scale.
FOLLOW UP FLAGS
This section enables practitioners to flag follow-up dates and required actions, which will appear in the ACTIONS section on the landing page.
