Completing the Athlete Medical Screening
You can complete the NIN Athlete Medical Evaluation (Screening) form via your AMS account on either your mobile APP or on a PC.
Once logged in, there is two ways to access this form:
- You will locate it on your landing page as a "Favorite Event", or
- Click on the Enter Data icon and locate and select the "NIN Athelte Medical Evaluation (Screening) form.
IMPORTANT
You will then be taken to the NIN Athlete Medical Screening form where you will need to select the version of the form that your NIN or NSO has instructed you to complete:
- Full Medical Screening, or
- Pre event/games/camp/annual review
Finally, once all required questions are completed, click the Submit but on the mobile APP or the Save & Close icon on the PC version.
