Concussion Return to Sport Protocol – AMS Checklist Guidance
When managing athletes under the Concussion Return to Sport (RTS) protocols, five key checkpoints must be completed before an athlete is cleared to return to competition:
Relative Rest
Light Aerobic Activity
Non-Contact Sport-Specific Training
Return to Contact and High-Risk Activities
Return to Competition
To support documentation and clinical decision-making across these stages, an AMS form has been developed to track athlete progress. The form allows practitioners to record start and end dates, progress athletes through checkpoints, hold them at a specific stage, or move them backwards if clinically indicated.

Access to this form and dashboard has been provided to the 2GEN_Medical and 2GEN_Physio roles across ASC and all CHILD sites. Access can also be granted to sport-specific Medical and Physio roles where required.

Date Calculations:
All automated date calculations are based on the Date of Injury recorded in the linked Injury Diagnosis entry.
Without a linked Injury Diagnosis entry, the form will not function as intended.

Starting a New Concussion Return to Sport Checklist:
Practitioners can initiate the checklist using either of the following methods:Option 1:
Create an Injury Diagnosis entry
Link it to a Doctors and Physios (Injury) Consult entry
Create a new Concussion Return to Sport Checklist entry from the consult
Option 2:
After creating an Injury Diagnosis entry, select Enter Data for Athlete
Locate the Concussion Return to Sport Checklist form
Begin data entry

Ongoing Data Entry:
For athletes already progressing through concussion protocols, the recommended workflow is to:Create a new Doctors and Physios (Injury) Consult entry
Link this consult to the existing Concussion Return to Sport Checklist
Form Behaviour and Data Corrections:
The form is designed to minimise scrolling. Once a checkpoint entry is completed, it will be hidden and the next selected checkpoint will be displayed.
If incorrect data is entered, practitioners must contact AMS Support to request corrections.
