UPDATED FORM LAYOUT: Doctor & Physio Consult Forms and Injury Diagnosis Forms
The Doctor & Physio Consult Form and Injury Diagnosis Form have been upgraded to improve visibility, reduce scrolling, and make common tasks more efficient. These changes will also be applied to the Doctor Consult and Illness Diagnosis forms.

What's Changed?
Improved Screen Layout
Information is now displayed across the page rather than in a single vertical column.

Historical Consult Notes Enhancements
Historical consult notes are now displayed alongside the current consult entry.
What's New
- Historical notes can be reviewed while entering current notes.
- Historical notes remain contained within their own scrollable section, reducing navigation and scrolling.
Copy and Paste from Previous Consults
The previous click-through requirement has been removed.
What's New
- Copyt and Paste text directly from historical notes.
- No longer do you need to go through to the record or copy notes to clipboard.
Required Fields More Visible
Mandatory fields are now highlighted more clearly.
Shared Communications Improvements
The Shared Communications section has been redesigned to provide clearer visibility of information being shared.
What's New
- Sharing options are expanded and easier to view.
- Fields available for sharing are open by default to clearly display the options and potentially reduce clicks.
- The note to be shared is displayed in Yellow for clear visualisation

- NO DATA IS SELECTED FOR AUTO-SHARING
- If athlete consent has not been obtained, set "Have you gained consent?" to No.
- If information is not being shared, select No for sharing details.
- You can choose specific fields and disciplines to share or share with all staff where appropriate.
Injury Diagnosis Form Improvements
The Injury Diagnosis form has been redesigned to improve visibility and navigation.
What's New
- Injury status is now more prominent and easier to locate.
- Related information is grouped more effectively.
- Additional sport-specific questions continue to appear automatically when applicable.
Faster Linking Between Consults and Diagnoses
The linking function has been moved to the top of the form.
What's New
- The 'LINK" button has been moved to the top, removing the need to scroll to the bottom of the form to access linking functions.
