PREVIOUS ACCESS TO KEY DATA ENTRY FORMS
INDIVIDUALISED DATA ENTRY METHOD
In the previous AMS workflow, practitioners must access separate data entry forms individually to input information for an athlete.
- This process is time-consuming, often requiring practitioners to leave one form to access another just to enter or retrieve relevant consultation data.
- Forms are stored in folders that can be difficult to navigate, leading to lost time searching for past records or locating the correct form for new data entry.
This highlights the need for a more efficient, centralized approach to streamline data entry and retrieval.
NEW ACCESS TO KEY DATA ENTRY FORMS
CENTRALISED DATA ENTRY METHOD
Practitioners can now:
- Seamlessly access key data entry forms directly within their discipline-specific Consult Form, allowing for efficient data entry across multiple locations in a unified workflow.
- Retrieve and link relevant pre-entered data from other forms within their Consult Form, ensuring that critical information is automatically consolidated into a single, comprehensive record.
This improves workflow efficiency and enhances collaboration across disciplines.
CONSULT NOTES AND INJURY DIAGNOSIS HAVE BEEN SEPARATED
To enhance the workflow, increase data security and enable better control over data sharing, the medical and physio consult notes and the injury diagnosis has been separated into two distinct forms.
By doing so, the consult form will now provide direct access to THREE options:
1. Show Historical Injury Diagnoses
- This allows practitioners the ability to easily review past injuries.
2. Link to an existing Injury Diagnosis
- Clicking "Link" allows you to connect the current consult entry to the relevant diagnosis records that it pertains to.
Updating Injury Status Using a Quick Edit
When linking a current injury diagnosis to a consult, a practitioner can use the pencil icon as a quick edit function to update or add a new injury status without having to navigate to, open and edit the injury diagnosis form itself.
3. Create a New Injury Diagnosis Record
- Start a New Record - Click the "+ New" button to open the Injury Diagnosis form.
- Complete the form - Fill in the required details
- Save Your Entry - Click "Save" to return to the current consult form.
Important Updates:
- The new Injury Diagnosis record now uses OSICS 15 codes, meaning diagnoses are selected from a database field instead of a body chart.
- Historical injury records with older OSICS codes have been re-coded using guidelines from discipline leads and uploaded into the new form for consistency.
- Original injury records will still be available for review if needed.
VIEWING SHARED CONSULT DATA FROM OTHER DISCIPLINES
Each discipline will have its own version of the new Consult Form.
All practitioners will have the ability to selectively share relevant information with other disciplines.
This section enables practitioners to review shared data from other practitioners while conducting their consultation, ensuring better collaboration and continuity of care.
VIEWING DATA FROM OTHER FORMS WITHIN THIS CONSULT ENTRY
This section of the new Consult Form workflow allows practitioners to easily access and navigate other forms or view stored data without leaving their consultation form.
In collaboration with discipline leads and working groups, each new consult form will include a selection of key and commonly used data entry forms. This enables practitioners to:
- Show Historical Information – View past entries from other relevant forms.
- Create a New Record – Enter data within a separate data form by navigating from within this form.
- Link Information – Attach relevant data from previously entered records.
ADDING AND LINKING TO DATA WITHIN OTHER FORMS

Create a New Record – Click "+ New" to open a data entry form, complete it, and click "Save" to return to the current consult form.
- Example: A Physician in the Medical discipline can seamlessly add a new allergy entry during their consultation.
LINK
Clicking "Link" allows you to attach relevant data from another form to this consultation. This helps create a more complete record and gives practitioners additional information to share with others when needed.
INCLUDING SUMMARY DATA FROM OTHER FORMS IN A CONSULT SHARED NOTE
Practitioners can now enhance their consultation notes by adding summary data from a new record or linking information from another form. This allows them to share more complete and potentially detailed communication notes with other disciplines, improving collaboration and patient care.
