Para Carer Consult
The Para Carer Consult form follows a standardised workflow used across all disciplines within the AMS. While each discipline has its own version of the form, the overall structure and process remain consistent. This approach helps practitioners work more efficiently, makes onboarding easier and supports collaboration between disciplines.
The consult workflow is the main documentation and communication tool used by practitioners on AMS. It serves as a central place to record athlete interactions, observations, assessments, recommendations and to share operational updates with other disciplines.
The Consult Form is designed to:
- Link consultation notes to relevant data and records stored elsewhere within the AMS to provide context and justification for notes.
- Provide access to key discipline-specific forms and data during the consultation process.
- Support the selective sharing of relevant information with other practitioners while maintaining privacy, confidentiality and appropriate data governance.
DATA ENTRY PROCESS
The process for recording Para Carer consults follows the same general workflow used across other disciplines. In most cases, the process will involve:
- Recording consultation details.
- Documenting consultation notes and observations.
- Linking or entering any relevant supporting data.
- Creating follow-up actions where required.
- Sharing relevant information with other practitioners when appropriate.
Not every consultation will require all components of the workflow. In many cases, practitioners may simply wish to record notes against an athlete profile without linking additional data, scheduling follow-up actions or sharing information with other disciplines. The form is designed to support both quick note entry and more comprehensive documentation when required.
DATA ENTRY WALKTHROUGH
1. ACCESSING THE FORM
The consult form will typically be available as a quick link under "Favourite Events" on the AMS landing page.

To start a new consult:
- Hover over the Para Carer Consult tile.
- Click "Enter Data"
- Search for and select the relevant athlete.
The form can also be accessed by clicking on the "Enter Data for Athlete" tab at the top of the page, selecting the athlete as above and then choosing the Para Carer Consult form from the list of available forms.
2. CONSULT NOTES
This section is used to document athlete interactions, observations, assessments, support requirements, recommendations and other relevant consultation information.
The form provides 5 fields for entering this information; a traditional SOAP style (in this case SOIP) notes taking format as well as a "General Notes" field that allows for quick note taking unrelated to any clinical observations. The use of the SOIP fields are suggested below:
Subjective: Clinical history.
Objective: Objective or physical examination.
Impressions: General impressions such as factors contributing to impairments.
Plan: Management plan for the athlete.

Notes should provide sufficient detail to support future reference, continuity of care and effective communication within the support team where appropriate.
3. VIEWING PREVIOUS & SHARED CONSULT INFORMATION
Each discipline has its own version of the consult form and can selectively share relevant information with other disciplines.
This section allows you to view previous consultation notes within the same record including:
- Consult information previously entered by either yourself or other Para Carers against this athlete's profile.
- Information that has been shared with Para Carers by other practitioners.
Having access to shared information during a consultation helps practitioners develop a more complete understanding of the athlete's current support needs and promotes coordinated multidisciplinary care.

4. LINKING OTHER DATA SOURCES TO THE CONSULT
A key feature of this workflow is the ability to link information from other forms directly into your consultation record.
Instead of copying and pasting information, you can now link to existing records such as:
- Evaluation records
- Travel reviews
- Attachments
- Any other relevant form
Benefits of linking:
- Provides richer context for other disciplines
- Reduces duplication of information
- Creates a more complete athlete record
- Allows you to enter data in its primary form first, then link it later
Important Workflow Tip: You do not need to be filling out a consult note to link information. You can complete an evaluation record or travel snapshot first, then open the Para Carer Consult form and link it. This is the recommended best practice.

Linking Records
Create a New Record
Create a new entry in another AMS form directly from within the Consult Form.
Select + New to open the relevant form, complete the required information and save the record. Once saved, you will be returned to the current consult record with all entered data still intact.
Example: A Para Carer may create a new Para Carer Attachments record to upload a PDF report while entering consult notes and comments concerning the report.
Link Existing Information
Select Link to attach an existing record from another AMS form to the current consultation.
Linking relevant information helps create a more complete consultation record and can provide useful supporting information when sharing consult summaries with other practitioners.
This functionality supports improved communication, collaboration and continuity of care across the athlete support network.
5. FOLLOW-UP ACTIONS
This section allows practitioners to record follow-up requirements, action items and review dates arising from the consultation.
Any follow-up items entered will appear within the Actions section of the AMS landing page, allowing practitioners to track outstanding tasks and upcoming reviews.
Examples may include:
- Reviewing an athlete's support requirements.
- Following up on equipment recommendations.
- Confirming implementation of agreed support strategies.
- Scheduling future athlete reviews.

6. SHARED COMMUNICATIONS
The Shared Communication section allows practitioners to determine what information is to be shared and who it is shared with.
The purpose of this section is to facilitate effective communication between relevant members of the athlete support team while ensuring information remains appropriate, relevant and consistent with athlete consent and privacy requirements.
Once athlete consent has been obtained, practitioners can choose to share:
- Consult note fields.
- Additional shared notes.
- Summary information from linked records and forms.
This allows practitioners to communicate key information without having to duplicate notes nor sharing unrelated or confidential details.
Please note that the decision to share information falls largely on the individual practitioner though expectations concerning sharing is governed by disciplines and programs aimed at ensuring the continuity of services to the athlete.
Sharing Information
After:
- Discussing information sharing with the athlete and obtaining consent; and
- Selecting the information to be shared,
the practitioner can choose the AMS roles that should receive the consult summary.
For example, information may be shared with practitioners in roles such as:
- Strength & Conditioning
- Performance Support Manager
- Coach
Shared information will only be visible to users who both:
- Hold one of the selected roles; and
- Have permission to access the athlete's profile.
Once sharing options have been selected, click Save and Close to save the consultation and distribute the shared summary to the nominated recipients.
