Para Carer Attachments
PURPOSE
The Para Carer Attachments form allows you to upload and organise important documents related to an athlete’s care. This includes reports, OT assessments, previous travel reviews, equipment prescriptions and any other supporting documentation.
The main purposes are to:
- Keep all relevant athlete documents in one central, searchable location.
- Provide context and evidence to support your clinical and operational decision making.
- Enable better continuity of care across the support team.
Important Note: Attachments themselves cannot be directly shared with other disciplines. However, you can share a description or summary of the attachment through a consult note. This also explains why you have access to multiple "attachments" forms as some are accessed across numerous disciplines:
CREATING A NEW ATTACHMENT RECORD
There are two common methods for creating new attachment records:
Via Favourite Events:
- Hover over the Para Carer Attachments tile.
- Click "Enter Data"
- Search for and select the relevant athlete.
Via Para Carer Consult:
- Open (or create) a Para Carer Consult record for the athlete.
- In the consult form, navigate to the Link To... section and find the Para Carer Attachments option.
- Click “New” to create a new attachment record.
UPLOADING FILES
Key Fields:
- Category – These categories help with searching and filtering. New categories will be added over time.
- Description – Provide a clear, concise description of what the document is (e.g. “OT Showering Assessment – March 2026”).
- Uploading the File
- Click on the attachment box or drag and drop the file into the box.
- Important: You must click the “Upload” button after selecting the file. The file is not saved until you click Upload.
NOTES & SHARING
- The actual file cannot be shared with other disciplines.
- You can share context by:
- Writing a note in the Para Carer Consult form.
- Linking the attachment record to that consult.
- Sharing the consult note with other roles.
This approach allows you to provide important context and results to other disciplines while maintaining file security and athlete privacy.
TIPS
- Upload files promptly and click “Upload” — many users forget or are confused by this step this step.
- Try to add a meaningful Description as often as possible — this is what other Para Carers will see.
- Categorise attachments accurately to make them easy to find later.
- Link attachments to relevant consult notes rather than leaving them isolated.
- Use the attachments form as supporting evidence rather than the sole source of information.
