Para Carer Evaluation
PURPOSE
The Para Carer Evaluation form is the main screening and assessment tool used by Para Carers to document an athlete’s support needs across multiple functional domains.
Its primary purposes are to:
- Capture a comprehensive picture of the athlete’s assistance requirements in daily living, travel and competition environments.
- Provide clear, actionable information for carers, coaches and support staff.
- Support travel planning, competition preparation and day-to-day care.
- Create a living record that can be updated over time and selectively shared with other disciplines.
This form is not a full clinical assessment — it is a practical support-planning tool designed specifically for the high-performance para sport environment.
LAYOUT & STRUCTURE
The form is organised into 10 main sections:
- Personal Care
- Mobility and Transfers
- Nutrition and Hydration
- Domestic Activities
- Medication Management
- Communication
- Cognitive and Emotional Support
- Social and Community Participation
- Safety and Risk
- Sleep and Rest
How the form works:
- Each section starts with high-level questions asking whether assistance is required across a list of domains.
- When you indicate that support is needed, additional fields automatically appear relating to that domain (e.g. specific tasks, time requirements, equipment, comments).
- This progressive disclosure keeps the form clean while allowing detailed information where required.
- Most fields use standardised options (e.g. Independent / Supervision / Minimal / Moderate / Full assistance) to ensure consistency across athletes.
- Where the predefined options don’t fully capture the situation, you can use the “Other” checkbox and/or the Comments / Notes field to add further detail.
COMPLETING THE EVALUATION
Accessing the form:
There are two common methods for creating new attachment records:
Via Favourite Events:
- Hover over the Para Carer Evaluation tile.
- Click "Enter Data"
- Search for and select the relevant athlete.
Via Para Carer Consult:
- Open (or create) a Para Carer Consult record for the athlete.
- In the consult form, navigate to the Link To... section and find the Para Carer Evaluation option.
- Click “New” to create a new attachment record.
Completing the form:
- Work through each section systematically.
- Only expand sections where the athlete requires support.
- Be as specific as possible in the task-level fields - this information will be summarised in dashboard reports.
- Use the Time Requirement fields to help with event planning and staffing.
- Add equipment details and comments where relevant.
- Always use the Notes fields for any unique context or important nuances.
REPORTING
Once saved, the Para Carer Evaluation generates several useful summaries:
- Concise summaries appear on the front page dashboard under the “PARA CARER EVALUATION” section.
- Each “flag” indicates that support has been identified in that domain:
- Clicking on a flagged cell opens a detailed view of the relevant information.
- A Travel Planning widget is also displayed, showing required equipment and extra time requirements broken down by time of day (Morning / Afternoon / Evening / Overnight).
- Again, clicking on a cell will provide you with a pop-up detailing where the extra time is requirement.
This allows staff to quickly understand an athlete’s needs without opening the full form.
TIPS
- Update the evaluation whenever there is a significant change in the athlete’s support needs.
- The form is designed to evolve — new options and fields may be added over time based on user feedback.
- Always consider consent and privacy when sharing information from this form.
- The more specific and accurate the information entered, the more useful it will be for the wider support team.
