PURPOSE

The Para Carer Travel Review is a form designed to capture and report on the specific support needs of an athlete when travelling for training camps, competitions or tours.

Its main purposes are to:

• Document the athlete’s practical support requirements during travel and away-from-home environments.
• Create a clear, shareable summary that can be used by support staff.
• Link directly to the broader Para Carer Evaluation and Para Carer Consult records for context.
• Support proactive planning (staffing, equipment, room setup, time allowances, etc.).

OVERVIEW

Accessing the form:

There are two common methods for creating new attachment records:

Via Favourite Events:

1. Hover over the Para Carer Travel Review tile.
2. Click "Enter Data"
3. Search for and select the relevant athlete.

Via Para Carer Consult:

1. Open (or create) a Para Carer Consult record for the athlete.
2. In the consult form, navigate to the Link To... section and find the Para Carer Travel Review option.
3. Click “New” to create a new attachment record.

Entering Data:

Step 1: Create a New Para Carer Travel Review

• Complete the relevant fields - refer to the field descriptions for advice on what to report on.
• Try to be concise; once all fields are combined the report has the potential to be very long.

Step 2: Construct the Report

• Note: The system automatically generates a long form summary report intended for Para Carers that includes all information entered - this is used for Para Carer reporting.
• To create a report that you are happy to share with other disciplines, select the individual fields from the "Sections" field to add to the report.
• As you add sections to the report, you can view the report being built in the section below.

Step 3: Save and Return

• Click “Save and Close”.
• IF starting this process from the consult form, you will return to the original Consult record with no data lost.
  • The Travel Review records is now linked to this consult.

Step 4: Finalise Sharing (Optional but Recommended)

• Link your consult record to the appropriate travel review record.
• Add any extra notes or link additional forms if relevant.
• Selectively share the Consult (which now includes the linked Travel Review) with other disciplines.
• Be sure to select the option that adds linked information to the shared note.
• Remember: Other Para Carers will automatically see the Travel Review — you do not need to share it with them.

REPORTING

• A long-form summary of the Travel Review is generated automatically.
• This is the version that is displayed on the frontpage dashboard for Para Carers.
