File Uploads via AMS Athlete App [Mobile]
Where to Access
Head to the My Data section in the AMS Athlete App to add an entry to your account.
Click the big Yellow Circle with a plus sign.
You will then find File Uploads form the available list of forms to you.
Select File Type
Currently the File Upload form only accepts both of the following forms;
1) PCR & Rapid Antigen Test
2) COVID-19 Vaccination Certificate
COVID 19 CERTIFICATE
1) Select the date of your most recent date of vaccination.
2) Upload File or Image. Finally select he plus button to either; take a photo of your certificate, find the image or photo in your mobiles photo library or file in the mobiles file library.
Be sure to press 'SUBMIT' when you have finished filling out the form to save the entry.
PCR & RAPID ANTIGEN TEST
1) Select the type of test conducted from the dropdown items including of PCR Test or Rapid Antigen Test types.
2) Select the date of the test by utilising the date field and scrolling through to the relevant day of month, month and year.
3) Select the Test Result. Positive, Negative or Pending.
Upload File or Image. Finally select he plus button to either; take a photo of your certificate, find the image or photo in your mobiles photo library or file in the mobiles file library.
Be sure to press 'SUBMIT' when you have finished filling out the form to save the entry.
File Uploads via AMS [Desktop]
Where to Access
Once you have successfully logged in to AMS and you are on your home page. Find the Data Entry Tab and press 'Enter Data for Athlete'.
Select the 'File Uploads' form from the list of forms you have permission to access. Tip: You can search for the forms name to find it easier if you have access to multiple forms.
Select the Test Type you wish to upload a file for. Select from COVID-19 Verification Certificate or PCR & Rapid Antigen Test.
COVID 19 CERTIFICATE
1) Select the date of your 2nd dose of vaccination.
2) Upload File or Image. Finally select he plus button to either; take a photo of your certificate, find the image or photo in your mobiles photo library or file in the mobiles file library.
Be sure to press 'SUBMIT' when you have finished filling out the form to save the entry.
PCR & RAPID ANTIGEN TEST
1) Select the type of test conducted from the dropdown items including of PCR Test or Rapid Antigen Test types.
2) Select the date of the test by utilising the date field selecting the relevant date from the calendar dropdown.
3) Select the Test Result. Positive, Negative or Pending.
Upload File or Image. Finally click on the box surrounded by the dotted line to then locate either the image/photo or pdf from your computers files.
Once you have located the file in the file window click the 'upload' button to upload the image.
A green tick will appear if successfully uploaded.
The files name and file type will then appear in the upload field. You can add multiple files if necessary.
Be sure to press 'SAVE AND CLOSE' when you have finished filling out the form to save the entry.
